MEDICARE REFERRAL

GP and Allied Health referral
Patients with chronic health conditions who are being managed by a GP on a Chronic Disease Management (CDM) Plan may be eligible for referral to an Avanti Exercise Physiologist under Medicare’s Enhanced Primary Care (EPC) scheme.
A chronic medical condition is one that has been (or is likely to be) present for six months or longer, for example, asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke.
An EPC referral entitles the patient to a maximum of 5 individual Medicare-subsidised sessions with an Allied Health provider in each calendar year. Depending on their condition, patients can be referred to multiple Allied Health providers to achieve the best team care approach for their health. Please speak to your General Practitioner to discuss your eligibility for an EPC referral - clients seeking Medicare rebates will need to have a valid referral form

A Medicare rebate is available for a maximum of five services per patient each calendar year.

Community-based patients may be eligible if they have a chronic medical condition and their GP has provided the following Chronic Disease Management (CDM) services:

A GP Management Plan (GPMP) – (item 721) and

Team Care Arrangements (TCAs) – (item 723).

Referring GPs may use one referral form to refer patients for single or multiple services of the same service type (e.g. 5 Exercise Physiology services).
Please note that Type 2 Diabetics may also be eligible to access 8 x group classes in addition to an individual EPC referral.
